Warfarin Sodium

An anticoagulant (blood thinner) used to treat and prevent blood clots.

Clinical information

Common uses

  • treating and preventing blood clots (DVT, pulmonary embolism)
  • preventing clots in atrial fibrillation
  • preventing clots with a mechanical heart valve
  • after a TIA or with a blood clotting disorder

Common side effects

  • bleeding more easily (the main side effect)
  • unexplained bruising or nosebleeds

Cautions

  • regular INR blood tests needed to check your dose
  • get urgent help for signs of serious bleeding
  • always carry your anticoagulant alert card
  • may not suit people with liver, kidney or bleeding problems

Interactions

  • NSAIDs such as ibuprofen or aspirin
  • antibiotics, antidepressants and antifungal medicines
  • St John's wort — do not take
  • vitamin K supplements — do not take
  • alcohol — avoid large amounts
  • cranberry and grapefruit juice — avoid

Pregnancy: not usually used in pregnancy — talk to your doctor
