Tacrolimus

An immunosuppressant that prevents rejection of liver, kidney and heart transplants.

Clinical information

Common uses

  • preventing organ rejection after liver, kidney or heart transplant

Common side effects

  • tremor
  • high blood pressure
  • abnormal kidney function
  • high blood sugar or new diabetes
  • diarrhoea, constipation and headache

Cautions

  • boxed warning: higher risk of serious infections and cancers
  • can damage kidneys — blood levels monitored
  • can raise potassium levels
  • avoid live vaccines

Interactions

  • grapefruit juice and nelfinavir — avoid
  • CYP3A inhibitors — raise tacrolimus levels
  • CYP3A4 inducers — lower tacrolimus levels
  • mycophenolate — exposure can increase
  • cannabidiol — may raise tacrolimus levels
  • other kidney-toxic drugs, e.g. aminoglycosides, amphotericin B
