Piroxicam

An NSAID capsule for the pain and stiffness of arthritis.

Clinical information

Common uses

  • osteoarthritis
  • rheumatoid arthritis

Common side effects

  • feeling sick
  • constipation or diarrhoea
  • stomach pain and wind
  • headache and dizziness
  • swelling (oedema) and rash

Cautions

  • boxed warning: raised risk of heart attack and stroke
  • boxed warning: stomach bleeding, ulcers and perforation, without warning
  • not before or after heart bypass (CABG) surgery
  • avoid with aspirin-sensitive asthma; monitor kidneys and blood pressure

Interactions

  • warfarin and other blood thinners (serious bleeding risk)
  • aspirin and other NSAIDs (more stomach harm, no extra benefit)
  • SSRI or SNRI antidepressants (bleeding risk)
  • blood pressure medicines — ACE inhibitors, ARBs, beta-blockers (less effective)
  • diuretics (reduced effect, kidney strain)
  • lithium, methotrexate, digoxin (levels or toxicity rise)

Pregnancy: avoid from about 20 weeks; do not use after 30 weeks
