Common uses
- schizophrenia in adults and adolescents 13-17
- bipolar depression on its own
- bipolar depression with lithium or valproate
Common side effects
- sleepiness
- restlessness (akathisia)
- movement problems (extrapyramidal symptoms)
- nausea
- weight gain and insomnia (children)
Cautions
- boxed warning: raised death risk in elderly dementia psychosis
- boxed warning: suicidal thoughts in young people — monitor closely
- monitor blood sugar, cholesterol and weight
- neuroleptic malignant syndrome and tardive dyskinesia can occur
Interactions
- strong CYP3A4 inhibitors (ketoconazole, clarithromycin, ritonavir) — do not combine
- strong CYP3A4 inducers (rifampin, phenytoin, carbamazepine) — do not combine
- St John's wort — do not combine
- grapefruit and grapefruit juice — avoid
- moderate inhibitors (diltiazem, erythromycin, fluconazole) — halve the dose
Pregnancy: third-trimester use may cause withdrawal symptoms in newborns
