Emtricitabine + Rilpivirine + Tenofovir

A three-drug combination tablet (emtricitabine, rilpivirine, tenofovir DF) as a complete HIV-1 regimen.

Clinical information

Common uses

  • initial HIV-1 therapy (viral load up to 100,000 copies/mL)
  • replacing a stable regimen in virologically suppressed patients

Common side effects

  • depressive disorders
  • insomnia
  • headache
  • diarrhoea, nausea and fatigue
  • dizziness, abnormal dreams and rash

Cautions

  • boxed warning: hepatitis B can flare severely after stopping
  • severe skin and hypersensitivity reactions including DRESS
  • severe depression — seek medical evaluation
  • kidney problems and bone density loss

Interactions

  • proton pump inhibitors like omeprazole — contraindicated
  • carbamazepine, phenytoin, phenobarbital — contraindicated
  • rifampin and rifapentine — contraindicated
  • St John's wort — contraindicated
  • antacids (take 2 hours before or 4 hours after)
  • azole antifungals and clarithromycin (raise rilpivirine levels)

Pregnancy: can be continued if already suppressed; viral load monitored closely
