Clozapine

An antipsychotic for schizophrenia when standard treatments have not worked.

Clinical information

Common uses

treatment-resistant schizophrenia
reducing recurrent suicidal behaviour in schizophrenia or schizoaffective disorder

Common side effects

sedation, dizziness, headache, tremor
fast heartbeat and low blood pressure
drooling, sweating, dry mouth
constipation and nausea
fever

Cautions

can cause severe white-blood-cell loss — regular blood tests (boxed warning)
fainting and low blood pressure during dose build-up
seizure risk, dose-related
heart inflammation (myocarditis) can be fatal

Interactions

fluvoxamine or ciprofloxacin — clozapine dose cut to one-third
carbamazepine and other strong CYP3A4 inducers — not recommended
tobacco smoking changes clozapine levels
anticholinergic drugs (toxicity risk)

Pregnancy: third-trimester use may cause withdrawal symptoms in newborns
