Clofazimine

An antimycobacterial medicine for lepromatous leprosy, always used in combination therapy.

Clinical information

Common uses

lepromatous leprosy, including dapsone-resistant cases
leprosy complicated by erythema nodosum leprosum

Common side effects

skin discolouration, pink to brownish-black (most patients)
dry, scaly skin
stomach pain, nausea, vomiting, diarrhoea
eye dryness, irritation and discolouration
discoloured urine, sweat and other body fluids

Cautions

can deposit in the gut — obstruction and bleeding can be fatal
heart rhythm changes (QT prolongation)
skin discolouration can cause depression — watch mental health
avoid with liver problems

Interactions

other QT-prolonging drugs, including bedaquiline
drugs metabolized by CYP3A4/5 (monitor for toxicity)

Pregnancy: only if benefit justifies risk — babies may be born pigmented
