Amiodarone

An antiarrhythmic tablet reserved for life-threatening recurrent ventricular arrhythmias.

Clinical information

Common uses

recurrent ventricular fibrillation
recurrent unstable ventricular tachycardia

Common side effects

corneal microdeposits (common, reversible)
raised liver enzymes
thyroid problems (over- or underactive)
photosensitivity and skin discoloration

Cautions

lung toxicity can be fatal (boxed warning)
liver toxicity can be fatal — regular liver tests (boxed warning)
can worsen arrhythmias — started under hospital supervision (boxed warning)
effects and interactions persist for weeks after stopping

Interactions

QT-prolonging drugs (avoid — dangerous rhythm risk)
warfarin (serious bleeding — dose reduced)
digoxin (halve dose or stop)
simvastatin and lovastatin (dose limits)
grapefruit juice (raises levels)
St John's wort (lowers levels)

Pregnancy: can harm the fetus — only if prescribed
