Bright yellow. Dark eye band with blue-white borders. Large black blotch on side, enclosed by a blue edged ocellus. Two blue-white lines passing down and backwards, one on either side of the pectoral fin, from a point above and beind the eye.
This species of fish is typically observed alone or in pairs, usually inhabiting outer reefs at depths ranging from 5 to 25 meters. Its diet consists mainly of coral polyps.

This species has a dorsal fin with 14 spines and 16-18 rays, an anal fin with 3 spines and 14-16 rays, and a pectoral fin with 14-16 (usually 15) rays. Its body depth is 1.4-1.8 times its standard length.
