Two distinct color forms of Monotaxis grandoculis have been observed, leading to speculation that two species may be involved; however, they are normally not caught by Maldivian fishermen.

Adults bluish grey above, paler below with centers of scales silvery and a few brownish scales generally present on sides. In life may appear either fairly dark and brown or pale and silvery. Juveniles white with 3 dark dorsal saddles; juveniles lose bars on preservation.
This species is often found near coral reefs at depths of 3 to 60 meters. It is nocturnal, meaning it is active at night, and feeds heavily on mollusks, crabs, hermit crabs, and sea urchins. It is usually solitary, but large adults may form small aggregations.

This fish has a dorsal fin with 10 spines and 10 rays, an anal fin with 3 spines and 9 rays, and a pectoral fin with 14 rays. Its body is oblong and has a depth of 2.1-2.7 inches in standard length, with relatively large eyes and a strongly convex head profile in front of the eyes. Additionally, its cheeks have several rows of scales, the inner base of its pectoral fins are scaled, it has molars present on the side of its jaws, and its maxilla has a longitudinal denticulated ridge.
